She's Inspired Book Club is a year long commitment and I am committed to engaging in group discussions as an active member

I am responsible for purchasing the decided books for the duration of the membership.

I am required to come prepared to each meeting having read the material.

Payment *

As a participant of She's Inspired Book Club, I acknowledge and agree to the following statements:

My payments will be made by either a. Monthly installments ($49.35); or b. Yearly installments ($446.25).

I will pay my registration fee by either a. (Monthly) By 1st of every month; or b. (Yearly) Before the first meeting in a new series.

I am aware of the commitment deposit ($49.35) required to be placed on the waitlist and understand this will be applied to my membership fee.

Media Release *

As a participant of She's Inspired Book Club, I acknowledge and agree to the following statements:

I consent to photos and/or videos being taken of me for advertising purposes for the duration of the She's Inspired Book Club.

I understand I am consenting to my photo being taken, and that I will be asked for verbal consent for the use of photos I am included in on a case by case basis.

Confidentiality *

This woman-centered group works at a deep and emotional level; some personal information may be disclosed. The integrity of our members allows for a free and open space for everyone to share their thoughts, feelings, and experiences. Confidentiality is key. Please note, no facilitator notes are taken during She's Inspired Book Club. As a participant in She's Inspired Book Club, I acknowledge and agree to the following statements:

I will hold strict confidentiality regarding all matters (names, events, etc.) pertaining to the personal work or identity of other participants in the group.

I will not disclose information about another group member outside of my group verbally, or through any written or electronic means including email, social media (Facebook, Instagram, Twitter, etc.), or any additional public forums.

If we are to meet remotely (i.e. video call facilitated by Zoom, etc.), I will maintain confidentiality by finding a private area during our meeting or use headphones with discretion.

I understand the facilitator is required by law to disclose confidential information in certain situations. I have read the confidentiality section in the membership information package and understand these conditions.

Although pictures of events may be used in marketing material, I may specifically ask to not have my photo published (even if I have signed the media consent).

If I have any questions about the potential impact of group discussions or the limits of confidentiality, I will contact the facilitator.
